Abstract

Indonesia's National Health Insurance system (JKN), covering more than 270 million participants in 2024, created a strategic paradox for partner hospitals: guaranteed patient volume on one hand, but fixed INA-CBG tariffs, tiered referral systems, and strict accreditation requirements on the other, demanding new competitive capabilities not accommodated by prior research models. This study aimed to develop and formulate an integrated conceptual framework explaining the roles of digital transformation, big data analytics, and organizational agility as antecedents of hospital competitive advantage, with service quality as a mediator and Islamic work ethics as a moderator. The study was designed using an explanatory quantitative cross-sectional approach with proportional stratified random sampling targeting 150 managerial and clinical respondents from six BPJS partner hospitals in Kuningan Regency, West Java, with data to be analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM) via SmartPLS 4. The framework produced nine hypotheses grounded in Dynamic Capabilities Theory, SERVQUAL Theory, and Islamic Work Ethics Theory, and extended a prior research model by replacing a static information-systems construct with three dynamic capability constructs. The study concluded that hospital competitive advantage under JKN depends on simultaneous investment in digital capabilities reinforced by Islamic work ethics as a conditional amplifier, providing a theoretical foundation and empirical validation agenda for hospital managers, BPJS Kesehatan regulators, and future researchers.
